Professional Statement

I attended the UNC School of Medicine and then completed my residency in pediatrics at Children’s Hospital Boston. I came to �����������/Texas Children’s Hospital (BCM/TCH) in 2005 to complete a combined fellowship training in Pediatric Emergency Medicine (PEM) and Global Health (GH), and joined the ����������� faculty in 2009. Currently I am director of the PEM – Global Health program at BCM/TCH. I have experience as a clinical researcher in global health program development and am an educator, both domestically and globally. I have breadth of experience and training in both pediatric emergency medicine and global health, including formal education in both tropical medicine and international disaster medicine. Past projects abroad have included direct patient care but have primarily focused on site development, including performance of site assessments and development of local partners, and subsequent capacity building of the sites through development of curricula, training programs, quality improvement projects and establishment of monitoring and evaluation systems. I focused the first 7 years of my faculty career collaboratively developing an Emergency Triage Assessment and Training (ETAT) training program in Spanish with the Pan American Health Organization aimed at building the capacity of hospitals in Latin America through implementation of high-quality, locally-relevant, sustainable training programs and triage processes for pediatric healthcare workers in-country. Most recently I spearheaded development of the Pediatric Alliance for Child Health Improvement in Malawi at Kamuzu Central Hospital and Environs (PACHIMAKE). This consortium brings together US-based institutions (����������� / Texas Children’s Hospital, Cincinnati Children’s Hospital Medical Center, University of North Carolina Chapel Hill / Project Malawi and the University of Utah / Primary Children’s Hospital) and local institutions (Kamuzu Central Hospital [KCH] Department of Pediatrics and the Malawi College of Medicine) to build pediatric emergency capacity and improve the care of acutely ill children at Kamuzu Central Hospital (KCH) through the implementation of effective, coordinated, high-quality clinical, educational, quality improvement and research initiatives. Though early in its development, we anticipate this consortium may serve as a model of collaboration for institutions striving to improve pediatric healthcare systems in other Low- and Middle-Income Countries (LMICs). In my current role as Director of the PEM-GH program at BCM/TCH I aim to develop initiatives that promote collaboration and foster partnerships between existing BCM/TCH Global Health programs, as well as domestic and global partners in global health education, health systems strengthening, care and treatment, advocacy, and research to improve emergency care of children worldwide. As a result of my previous global and educational experiences, I am acutely aware of the importance of collaboration and partner development at every level.

Pediatric Alliance for Child Health Improvement in Malawi At Kamuzu Central Hospital and Environs (PACHIMAKE)

Project Aim • Through a sustainable, bi-directional collaboration, improve the care of acutely ill children at Kamuzu Central Hospital (KCH) through the implementation of effective, coordinated, high-quality clinical, educational, quality improvement and research initiatives Project Objectives • Create and sustain a collaborative group to build capacity for pediatric care focusing on initiatives related to Pediatric Emergency Medicine (PEM) within the KCH Department of Pediatrics, recognizing that PEM is a pediatric subspecialty with the potential to serve as both the entry point and safety net for the continuum of pediatric care for children in Malawi. Institutional members of the consortium include: BCM/TCH PEM, BCM Children’s Foundation and Center of Excellence, Cincinnati Children’s Hospital Medical Center, University of North Carolina Chapel Hill / Malawi, University of Utah / Primary Children’s Hospital, Kamuzu Central Hospital (KCH) Department of Pediatrics and the Malawi College of Medicine (COM). • Conduct yearly site assessments on ground with all stakeholders: November 2016, November 2017, January 2019, January 2020 • Prioritize and address the following goals focused on known gaps to improve pediatric outcomes over next 10-15 years: o Access: To improve pediatric emergency medicine outcomes at KCH o Capacity Building: To bi-directionally augment the ability of pediatric providers to care for acutely ill children at KCH o Partnership: To strengthen coordination and management of PACHIMAKE partnership to provide impactful services at KCH o Improved infrastructure: To ensure adequate infrastructure (physical and services) at KCH to provide care for acutely ill children Individual contributions: • Developed concept of the consortium and facilitated agreement between institutions to collaborate • Faculty lead for program including collaborative oversight of all clinical, educational, quality and research initiatives and scheduling • Develop and maintain monthly staffing calendar for consortium including all rotating faculty and trainees • Worked clinically at KCH in Emergency Zone building capacity supporting for acute care pediatrics June 2019 • Arranged yearly meeting with all key stakeholders in Lilongwe from 2016 – present to define key gaps, propose short, medium and long term solutions and strategize consortium approach • Lead investigator on TCH Global Health Grant submitted March 2017 and private donor presentation February 2019 • Developed internal program tracking tool to monitor PACHIMAKE outputs and impact of those activities • Developed logo for consortium Project Outcomes (pending): • Increased presence of PEM and general pediatrics faculty/fellow in Malawi since 2014. Academic year 2016- 2017 with 22 cumulative months, 2017- 2018 with 36 cumulative months, 2018- 2019 with 32 cumulative months, 2019 – 2020 with 22 cumulative months (an additional 14.5 cumulative months were lost due to protests in Malawi and COVID) at KCH Department of Pediatrics in high-acuity areas focusing on: o Clinical bedside teaching of permanent staff, medical students, clinical and medical officers and nurses o PEM didactic teaching to all levels of healthcare personnel o Defining priority educational and process initiatives to be developed • Hired full-time general pediatrician to work with PACHIMAKE at KCH starting August 2017 - current • Consortium support of COM to begin pediatric residency program at KCH starting fall 2020 • Developed coordinated scheduling calendar for US-based institutions and instituted monthly phone calls for all stakeholders • Developed interim database implemented in Department of Pediatrics 8/17 to collect data which will inform priority interventions • Developed standardized orientation packet for all PACHIMAKE US-based rotators at KCH to ensure consistent care • Supported locally-led ETAT trainings at KCH July 2017 which trained 8 new facilitators and 80 participants and continues yearly • Performed collaborative site assessment November 2016 and follow-up visits yearly • Since 2016, 6 collaborative manuscript published and 14 abstracts presented at national conference • Grant applications submitted x 2 • Private donor requests successful x 2: Schmidt Family Foundation $17,800/year x 5 years (2020), Peacock Family Foundation $100,000 (2020)
